This is the consent form signed by every shopper recruited to wear Shopnosis camera glasses during a research session. It is provided here for transparency — the actual signed copy is collected on paper or digitally at the start of each fieldwork session.
Consent form
Shopnosis BV is an independent market research organisation that has been commissioned to conduct research on shopping habits. We are conducting sessions with people who represent the general public.
During the session we will record shopper view data for the purpose of the study.
You may be asked some questions before and/or after the session, as well as to produce your check-out receipt for our review and note-taking.
The research data gathered from this study may be published in a form that does not personally identify you.
By signing this form, you are giving your consent for us to use your statements and images of view tracking. They will be treated confidentially and will not be distributed to anyone, for any purpose other than Shopnosis’ and its clients’ research and learning sessions.
You are free to ask any questions or withdraw at any time from the session. If you experience any discomfort wearing the glasses during your shop, please feel free to return the glasses to the front of store.
Your or your family’s full names or any personal information or contact details will not be linked to the study and will not be available to anyone.
